I have changed my name after 12th, but my old passport has my new name. Do I need to provide any document while applying for reissue of passport? I understand that I have to check yes in section where it asks if you ever changed your name and give my old name.
If your current/expiring passport already has the updated name, you dont need to provide any document for renewal.
