H1B transfer after max out with cool down period

Is this true and doable ?

left USA before 6 months to max out and staying in india to complete 365 calendar days away from USA.

Am I eligible to go for H1B renewal/extension/transfer after 365days cool down to get renewed for another 3years !!

is this true and can happen without PERM i140 filed at all ?

No. If there is no PERM & I140, after spending 365 days outside of the US, your new H1B petition will have to go via lottery (cap-subject) again.
