Do I need to fill i539A form if filing h4 extension for 6 yr alone?

  1. I am applying h4 extension for my daughter(6 years) alone, do I need to fill i539A form?
  2. Do I need to fill interpreter or preparer or both information with my name?

You can only file form i539. I539A is for additional applicants on same i539 application.
