You can use “father” if you were born in India and you are Indian citizen.

Hi… In this case what would SELF indicate? If you were born in India, then became a US citizen… and now you are filling out the OCI form then you could use SELF?

Yes @Ravi91502, self means you are filing application for yourself.

What option can we pick when we are filling out the Spouse’s details? There is no option that says, Spouse.

I am preparing this form for my wife, here husband option is not available…
what to fill?

my wife has middle name; but in the online form; only surname and given name available;
once we genrate the final form middle name appear… how to enter middle name…?
