This would be $75 in most cases.

they mention “Enter the amount of fee excluding Ckgs fee”

Actually, it does not really matter. People have been writing the full amount including me (in Jan 2018) and we got the passport renewed with no issue.
This is a pretty old format when they used to collect full payment by one check.
You can either write the full amount or just the $75 passport fees.
Your choice.
