Notary does not sign on the document but attach a paper with their stamp and signature

I live in the State of California and here in California the notary public does not sign on any documents if it is not in their format.

The Notary whom I met gave me a California Jurat with her signature and attached it to the Change of appearance affidavit.

That’s fine. Many states follow this kind of procedure.

Just send it along with all other documents to CKGS.
