H4 EAD renewal along with multiple H4 extensions

Hi Anil,
I do want a file for the H4 EAD renewal along with an H4 extension (spouse and 2 kids).
My elder son has 14+ years old. In his H4 extension document, Do I need to ask him to do the signature or my spouse can do the signature?
Do I need to send the documents separately or Can I send all the documents (H4 EAD renewal + H4 extensions documents(spouse+2 kids) in a single package?
If yes, should I clip the documents of each file?
